Not an actual simulator; this is a parody of various simulation programs (train simulators, farm simulators, flight simulators, etc.), which is based around having an intentionally-clunky way of interacting with the game world. The player uses their hand in-game to pick up and manipulate objects; your wrist and arm movement are controlled with the mouse, while your fingers and thumb are controlled with the keyboard. The hand (belonging to the "main character", Nigel Burke) is very inaccurate and inflexible, which results in surgical operations that are very easy to botch, often in hilarious ways.
Originally released as a free game in January 2013 with only a heart transplant mission, Surgeon Simulator 2013 received an upgraded Steam release in April. The Steam version has received a number of free updates since its release, including an extra operation which ties in with Valve's Team Fortress 2, and some super-secret hidden levels which require one to either work out a code through obscure in-game hints, or look up the answers online.
